Welcome to the KIPP Texas Public Schools – Houston student-athlete online registration portal!

• To access the online forms, click the “Start Online Forms” button below.

• If you have previously created an account, you may sign in and complete the forms. If you have not previously created an account, you may create one using your personal email address. If you do not want to create an account, click “I don’t want an account. Go to forms.”

• Once you have filled-out all of the information, you will have the opportunity to print the document.

• Please be sure to complete all the tabs (at the top of the page), working your way from left to right. Each tab has required paperwork that needs to be completed on an annual basis. To sign the document click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box. Once ALL documentation has been completed and received, your student-athlete will officially be eligible to participate in interscholastic athletics. Below are the documents included in each tab:

Home/Instructions
This is the tab you are currently on – all instructions are included. You can refer to this at anytime during the registration process – Simply click on the “Home/Instructions Tab”.

Fill and Complete
In this section you must complete all required information. Information you must provide includes:
* Emergency Contact
* Medical History
* Sports Participation Survey

Once you have provided all required information, the student and parent/guardian must sign, date, and click “submit” at the bottom of the page.

Read and Acknowledge
In this section, you must click on each document provided and read thoroughly. Once you have read the document, check the box next to it. Once you have thoroughly read all documents and have checked all the boxes, the student and parent/guardian must sign, date, and click “submit” at the bottom of the page. Items in this section include:
* HCAL Manual Acknowledgement
* Student-Athlete Handbook Acknowledgement
* Permission to Participate and Travel
* Assumption of Risk, Liability Release, and Indemnity Agreement
* Concussion Acknowledgement
* Anabolic Steroid Use Agreement
* Sudden Cardiac Arrest Awareness

Download, Print, Complete, and Return
In this section, you must download, print, complete, and return the Pre-Participation Physical Evaluation – Medical History form to your Campus Athletic Director. You only need to complete the Legacy Health Consent Form if you are having the physical evaluation performed by Legacy Health. If so, please take this form with you and provide to Legacy Health at the time of your physical.
